The spouse was cared for at home with support from the district nursing team and Macmillan nurses during the final months. The patient is registered with your GP practice and the GP has referred the patient for a bereavement support check-in. The patient has no significant physical health problems but has been struggling with sleep and appetite since the death.

•Greet the patient warmly and express sympathy for their loss. Use the deceased spouse's name. Acknowledge the significance of 48 years of marriage. Create space for the patient to share their experience.
•Explore how the patient has been coping since the death: sleep, appetite, energy, concentration, social engagement. Differentiate between normal grief (waves of sadness, yearning, loss of interest, sleep/appetite disturbance) and complicated grief or depression (persistent intense yearning, feeling life is meaningless, inability to accept the death, prolonged functional impairment beyond 6 months).
•Assess social support: family nearby, friends, community groups, religious or spiritual support. Explore whether the patient has been able to talk about their spouse with others.
•Provide psychoeducation about the grieving process: grief is not linear, there is no 'right' way to grieve, everyone's timeline is different. Normalise common experiences (talking to the deceased, dreaming about them, feeling their presence, 'grief bursts' triggered by reminders).
•Discuss practical support: bereavement counselling (Cruse Bereavement Support), local bereavement café or support group, GP follow-up. Provide information about what to do if grief becomes overwhelming (urgent support contacts, including Samaritans). Offer a follow-up appointment in 4-6 weeks.

•When asked, say your daughter lives 45 minutes away and visits at weekends. Your son is abroad. You don't want to burden them.
•Say you sometimes talk to your spouse's photo and you dream about them almost every night. In the dreams they're alive and well. You wake up and the realisation hits you all over again.
•Say you're not sure if talking will help but you're willing to try. You'll think about the bereavement café. You'd like to come back and see you again.

That is completely normal. Many people who have lost a loved one experience moments where they forget for a split second that they have passed away. These moments can be very painful but they are a normal part of the grieving process as your mind adjusts to the new reality.

Normalise common grief experience. Provide validation and explanation.

Do these feelings of sadness ever go away completely?

She was the kindest person you would ever meet. A primary school teacher. The children adored her. We retired together travelled together gardened together. Now the house is so quiet.

Nurse

She sounds like a wonderful person. The quietness of the house after losing a lifelong partner is something many people find hardest — all the shared routines that suddenly are not there anymore. How have you been managing day to day?

Validate the specific pain of lost companionship. Assess daily functioning.

Patient

Not well if I am honest. I cannot sleep. I lie awake thinking about her. I have lost my appetite and lost about half a stone. I do not feel like going out or seeing anyone.

Nurse

Those are very common physical responses to grief — poor sleep appetite changes weight loss and social withdrawal. These do not mean something is wrong with you. They mean you are grieving a significant loss. Have you spoken to anyone about how you are feeling? Family or friends?

Normalise physical symptoms of grief. Assess social support network.

Patient

My daughter lives forty-five minutes away. She visits weekends. My son is abroad. I do not want to burden them.

Nurse

It is very natural to want to protect your children. But I want you to hear this: you are not a burden. Grief is heavy enough to carry alone and sharing it with people who love you is letting them support you. Have you thought about a bereavement support group?

Directly counter the guilt of being a burden. Introduce support group idea.

Patient

I am not sure. I have never been one for group things.

Nurse

I understand and there is no pressure. There is a bereavement cafe that meets at this centre every Tuesday morning. It is very informal — just tea and conversation with others who have lost a spouse. Some people find it helpful to talk to people who truly understand. You can just come and listen if you prefer.
